CMS is proposing major changes to the Home Health Value-Based Purchasing (HHVBP) Model in the CY 2026 Home Health Proposed Rule.
Here’s what you need to know:
Measure Removals
– CMS plans to remove three HHCAHPS-based measures:
– Care of Patients
– Communications Between Providers and Patients
– Specific Care Issues
Why? These measures rely on survey questions that are being revised starting April 2026.
New Measures Proposed
– Three OASIS-based measures:
– Improvement in bathing (M1830)
– Improvement in upper body dressing (M1810)
– Improvement in lower body dressing (M1820)
– One claims-based measure:
– Medicare Spending per Beneficiary for Post-Acute Care (PAC)
Proposed Category Weight Changes
– Larger cohort:
– OASIS-based measures – 40%
– Claims-based measures – 40%
– HHCAHS survey-based measures – 20%
– No proposed changes to the smaller cohort category weights.
What It Means for Agencies
– A shift toward functional outcomes and cost efficiency
– Less reliance on patient surveys, more emphasis on clinical documentation and claims data
– Agencies will need to tighten their OASIS accuracy and monitor spending metrics closely
